FMSW's physician faculty of
13 includes 9 family physicians with OB privileges, one Internist who
is board certified in Endocrinology, one PharmD Pharmacist, one
Behavioral Scientist, and two Physician Assistants. The Family Medicine
Center is a 18,500 sq.ft. clinic, and resident "home base". It serves 12,500
patients. While clinic patients represent all walks of life, approximately 70
percent are Medicare, Medicaid, underserved, disabled, Latino, and
Russian-speaking segments of our community. FMSW faculty and residents provide
36,000 outpatient and 7,000 inpatient visits annually, in a successful,
innovative Advanced Access care model, with team-based chronic disease
management programs for Diabetes, Asthma, and Medical Management of Chronic
pain. We also do diabetic group visits. With the anticipated "go live" of GE
Centricity EMR in the early Spring of 2009, FMSW will have completed integrating
all the elements of the TransforMed Medical Home model (http://www.transformed.com/). Our
residents have been trained in the "New Model" since 2004 when we launched our
