TREATMENT SUGGESTIONS FOR OPIATE WITHDRAWAL
Clonidine 0.1 mg 1-2 tabs PO Every 4 hours PRN withdrawal
Phenergan 25 mg 1-2 tabs PO Every 6 hours PRN nausea
Trazadone 50 mg 1-3 tabs PO at bedtime
Hydroxyzine 50 mg 1-2 tabs PO Every 4 hours PRN anxiety
Imodium Over the counter. For diarrhea. PRN. Follow package
INSTRUCTIONS
These medications may be continued for anywhere from one week to 60 days. The length of time needed will be related to the amount and type of narcotic used to create the addiction.
Maintaining hydration is VERY IMPORTANT!!!
Suggest the following recipe: 1/3 part Gatorade (any flavor) 2/3 part Water Consume throughout the day.
To wean from narcotics, decrease the amount of narcotic (if the patient still has narcotics) by 5% a day.
Do NOT add benzodiazepines to the patient's medications.
If the patient is taking benzodiazepines currently, do not decrease the dose or discontinue their use. (The reduction of benzodiazepines may be done very slowly in the future).
Never mix Suboxone and benzodiazepines.
Do NOT prescribe Soma.
Refer patient to a center for addiction medicine or detoxification or to a Suboxone provider.
