You are entitled to receive copies of your record. To protect your privacy, you will be required to provide proof of identity. There may be fees. To obtain a copy of your records:
Certified birth or death certificates are issued by the State of Washington. You may order them online at http://www.doh.wa.gov/ehsphl/chs/cert.htm or call (360) 236-4300 and select "0" to speak to the Customer Service Representative. The mailing address is Washington State Department of Health. 1112 SE Quince Street, P.O. Box 47890. Olympia, Washington, 98504-7890.
