Solutions Employee Assistance offers evaluation services to those who are troubled by their use of alcohol and other drugs*. If you like, take the following survey to see if you may have a problem with alcohol or other drugs.

Are your family members or friends concerned about your drinking/use?
Have you recently been cited for DWI, MIP, or any drug charge?
Do you sometimes worry about how much you drink or use?
Do you drink and drive?
Do you continue to drink/use despite family, social problems being worsened by your alcohol or drug use?
Do you often drink or use more than you intend to?
Do you drink or use despite having physical or psychological problems that are either caused or made worse by your doing so?
Have you tried to cut down or stop for periods of time, but been unable to for very long?
Have you missed important events because you were drunk or high?
Have you missed work because you were drunk or high or because you had a hangover due to use the night before?
Do you sometimes feel guilty about your drinking or use?
Do you drink/use in the morning to get yourself going for the day?
Do you drink in order to relax or relieve stress?
Do you need to use more alcohol/drugs now to get drunk or high than you did even a few months ago?
Are you irritable when you go without alcohol or drugs for a period of time?
