  • Apply on-line for the "Pharmacy Resident" position at www.swmedicalcenter.org
  • Find the position in the job list under "Employment" and "Employment Opportunities"
• Copy of curriculum vitae
• Personal Goal Statement describing reasons for wanting pharmacy practice residency training
• Three letters of recommendation
• Official copies of all college transcripts
